HOURS : 30 per week, flexible times, qualifies for full-time benefits

LOCATION : Hybrid; in office during orientation

JOB SUMMARY : Provide comprehensive case management services for seniors and individuals living with disabilities that are enrolled into MSHO / MSC state health plans.

RESPONSIBILITIES :

  • Model and foster behavior that establishes a culture of diversity, equity and inclusion.
  • Manage a caseload of clients who are 65 and older who are primarily on the Elderly Waiver.
  • Regular communication with client, care team, providers, and family members

  • Complete initial and annual psychosocial assessments in clients homes to identify presenting problem(s) and gather pertinent history
  • Complete other necessary paperwork and documentation on a timely and regular basis
  • Create unique care plans based on assessment / reassessment identifying needs, goals, and interventions.
  • At a minimum, complete bi-annual check ins virtually and update the clients care plan
  • Complete detailed and timely case notes on every interaction with clients and families / professionals involved in their care.
  • Initiate, facilitate, and maintain home care-based services for clients
  • Collaborate with family and others from the clients support system to ensure clients needs are met
  • Collaborate with medical professionals and care teams, as needed
  • Make referrals to other agencies or resources as appropriate.
  • Assist with transitions and discharge planning when a client is returning home from the hospital or nursing home.
  • Utilize interpreters for effective communication with clients from various backgrounds
  • Participate in mandatory in-person team and staff meetings and case consultation discussions.
  • Participate in mandatory 1 : 1 meeting with direct supervisor (frequency and method of contact are flexible)
  • Participate in health plan and DHS trainings and meetings, as required
  • Provide coverage for other members of the care coordination team, as assigned
  • Adhere to health plans procedures and regulations
  • Follow agency procedures for keeping and maintaining electronic client files
  • Other duties as assigned
  • OTHER FUNCTIONS :

  • Adhere to NASW Code of Ethics, personnel practices and other designated policies and procedures.
  • Pursue professional growth through reading, workshops and other learning opportunities directly relevant to the above-described job responsibilities.
  • Participate in JFS committees as desired

    MINIMUM QUALIFICATIONS :

  • Commitment to fostering an equitable, inclusive, and culturally relevant environment for JFS employees, volunteers, clients and donors.
  • Licensure as a Social Worker through the Minnesota Board of Social Work (LSW, LGSW, LISW, or LICSW) or as a Registered Nurse / RN through the Minnesota Board of Nursing
  • Bachelors degree in social work, Psychology, Sociology, Nursing, or within the human services field
  • Valid driver's license and a personal vehicle to use for agency business.
