JOB DESCRIPTION
Position Title: Intensive Case Management Specialist
Program: New American Program
Status: Full-Time, Non-Exempt. Position is temporary through September 2025 and is contingent on continued PC funding.
Reports to: Initial Services Coordinator
Purpose: To coordinate intensive case management and develop meaningful partnerships and referral networks to support newcomers.
Responsibilities:
· Manage active caseload of refugee and newcomer clients; prioritize urgent appointments, maintain transparent and consistent communication, and connect clients to community resources and institutions.
· Advocate for clients and help them navigate housing, employment, benefits, and education systems.
- Provide multi-month case management and community orientation services to support the self-sufficiency and independence of newly arrived clients.
- Conduct thoughtful service provision in line with the requirements of the Preferred Communities program, including program orientation and enrollment, assessment of needs, determination of program eligibility, and creation and implementation of a detailed Self-Sufficiency Plan for assigned clients.
- Maintain accurate, up-to-date case notes and programmatic documentation.
- Conduct financial assessments to determine client’s need for assistance; transparently and fairly provide grant funds for stabilization services.
· Transport or secure transportation for clients to appointments as needed.
· Build the capacity of the agency and external partners to serve refugees and newcomers with particularly acute needs who can benefit from intensive case management services.
· Consult with department coordinators within the New American and Behavioral Health departments about referrals and client progress.
· Assist with preparation and timely submission of program reports to funders.
· Perform all duties within the framework of the agency mission and ethics.
Qualifications:
· Demonstrated experience providing orientation, education, and case management to vulnerable refugee and immigrant populations.
· Demonstrated experience working from a strengths-based perspective to support clients in navigating systems; connecting with community institutions; and obtaining needed services and benefits.
· Fluency in Arabic, Swahili, Kinyarwanda, or Dari preferred; experience using interpretation services highly preferred.
· Enthusiasm for supporting clients of a variety of cultural, religious, economic, gender identity, and educational backgrounds.
· Bachelor’s degree or equivalent experience.

- Outstanding verbal and written communication skills.
- Valid driver’s license with regular access to a reliable vehicle.

Pay: $22.00 - $25.00 per hour
Expected hours: 37.5 per week
