MDS Nurse - Per Diem

This is a Per Diem, hourly MDS Nurse position for an experienced MDS Nurse in a 141 bed Skilled Nursing Facility. The schedule is
The MDS Nurse is responsible for the following:
Coordinating the Resident Assessment Instrument (RAI) and the individualized plan of care for all patients/residents.
Ensuring all residents are assessed and reviewed in a timely manner using the RAI process, following MDS regulations requirements per the RAI manual, coordinating the notification of staff and family.
Completing MDS’s, CAA’s, Care Plans, and resident interviews on short term rehabilitation residents.
Assisting with the education of the nursing staff as well as documentation skills.
Must be dependable, reliable, team player with a positive attitude and a good prior work history. Additionally, they should have MDS experience, a current MA license, LPN or RN (preferred), certification preferred but not required.
