Job Overview
The Occupational Therapist will complete screening, evaluations, re-evaluations, and discharges as supervised by the Physician. The Occupational Therapist will provide supervision to Occupational Therapy Assistants as directed by the Director of Rehabilitation. PER-DIEM
Responsibilities and
Duties:
• Evaluates patients assigned per MD
orders and as indicated, and establishes appropriate Plan of Care and Goals
that meet the specific needs of the individual.
• Re-assesses patients and completes
Updated Plan of Care least every 30 days, or as indicated on initial
evaluation, in order to determine need for continued services or
discontinuation of services based on patient progress. Carry out treatment plan
as indicated.
• Screens all patients in facility on
a quarterly basis, to identify those who have experienced declines in their
functional abilities and determine if Occupational Therapy intervention is
indicated.
• Provides thorough discharge
planning that will allow the patient to function at a maximal level of independence
in the least restrictive environment.
• Observes and records patient’s
progress, response to treatment and limitations and maintains this information
in electronic medical record.
• Completes
required documentation at point of service.
• Supervises
Occupational Therapy Assistants as assigned by Director of Rehabilitation.
• Identifies needs for program
development and implements programs according to company policies and
procedures.
• Coordinates in-services for
facility staff as need is determined, to ensure safe and effective carry over
of Restorative Programs and overall call of the residents.
• Attends and reports accurately a
patient’s status at weekly UR meeting and interdisciplinary care plan meeting.
Qualifications:
• Education:
Graduate of an accredited therapy program
• Licenses/Certifications:
Current Occupational Therapist New Jersey state license
• NBCOT
Certified
At least 5 years of LTC experience required
