Position Summary : Case Managers provide individualized support and ensure comprehensive delivery of services to participants engaged in education efforts related to healthy relationships and the effects on children's outcomes. The goal is to empower individuals and families towards improved self-sufficiently through coordinated case management. Services include identifying needs, tracking progress, addressing barriers, referrals to community resources as well as gaining and maintain employment and financial stability.Specific Tasks and Responsibilities :
- Serve as initial contact to referrals, establish relationships, recruit, assess and develop individualized service plan with family.
- Provide comprehensive case management services for participants.
- Develop strong therapeutic relationship with families.
- Create and maintain treatment plans
- Monthly contact with participants
- Apply for funding in accordance with the client's needs
- Document client information, service delivery, and other information as required by JFCS and funder.
- Track pre and post surveys as required by the grant
- Ensure participants complete required surveys
- Recruit and retain referral sources and participants.
- Actively participate in Department and JFCS Outreach activities.RequirementsEducation : A bachelor's degree in behavioral health or related field with a minimum of two years direct social service experience.Qualifications :
- In-depth knowledge of and proficiency in the Microsoft Office Suite (Outlook, Work, Excel, PowerPoint)
- Knowledge of the role of social, economic, and environmental factors in the development and resolution of person and family problems
- Strong ability to relate to other professionals and lay people in the community
- Capacity to form and maintain relationships with participants, perform home visits, and mobilize individual and / or family strengths in alignment with community services and supports
- Sensitivity to service populations and socio-economic issues.

- Strong knowledge of community supports and resources.

This position requires full range of motion, including manual and finger dexterity and hand and eye coordination. This position requires seated work at desk, including use of a computer. This position requires the ability to lift 30 pounds. This position requires the ability to drive and have regular access to a reliable automobile for travel to remote offices. This position requires working under stressful conditions or working irregular hours.Required Training :
- Complete all JFCS and Healthy Families / Healthy Children mandatory trainings, including Orientation and all trainings due within the first 6 months of hire.
- Obtain all annual JFCS required trainings related to COA standards, TIER, HIPAA, and Security Awareness.
- Training on proper documentation techniques and the maintenance and security of case records and personnel files.
