The Heart Failure Nurse Practitioner at Raritan Bay Medical Center will deliver primary health care based on individual credentialing. The successful candidate will work with private physicians to enhance throughput and decrease length of stay, perform patient teaching to decrease readmissions, and perform continuity of care phone calls with skilled nursing facilities. This Nurse Practitioner will work closely with floor nurses, social workers, case managers and physician advisors as part of the patient care team.
Schedule: 5 days a week with weekend obligation.

Responsibilities
- Provision of health care across the health care continuum for the promotion of health, prevention of disease, and the early detection and management of episodic symptoms of acute and chronic health problems.
- Elicits a complete medical history and documents appropriately in the progress notes.
- Conducts an appropriate physical exam and documents appropriately in the progress notes.
- Performs diagnostic and therapeutic procedures (As permitted from credential committee).
- Orders and interprets diagnostic testing as indicated by the patient's health care needs.
- Devises and implements a patient management plan in direct consultation with a physician.
- Orders pharmacologic agents in direct consultation with physicians (as detailed by credentials and privileges).
- Provides competent assessment, treatment and/or care to patients appropriate for the individualized and developmental needs for the ages of those patients served.
- Participates as an active member of the multidisciplinary team cooperating in the planning and development of patient care needs and program needs.
- Serves as an active member on Medical Center Committees as appointed or assigned.
- Attends monthly department and division meetings.
- Participates in the daily multidisciplinary patient care team meetings and weekly long stay rounds.
- Performs duties requiring a licensed independent provider in through put, length of stay reduction, readmissions reduction, and patient experience improvement.
- Other duties and/or projects as assigned. 15. Adheres to HMH Organizational competencies and standards of behavior.
Education, Knowledge, Skills and Abilities Required:
- Masters Degree in Nursing.
- Advanced Practice Nurse National Certification;
- Unrestricted license to practice in New Jersey. Eligible for credentialing and Appointment to Medical Associate Staff.
- Minimum of 3 to 5 Years Acute care experience.
- One year experience as Nurse Practitioner.
- Excellent written and verbal communication skills.
- Proficient computer skills including but not limited to Microsoft Office and Google Suite platforms
Licenses and Certifications Required:
- NJ State Professional Registered Nurse License.
- Advanced Practice Nurse Cetification (w/ Master's Degree).
