We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. Responsibilities
- Assess patients' needs and develop individualized care plans based on diagnostic evaluations.
- Coordinate with multidisciplinary teams to ensure effective patient management across various settings including primary care, outpatient services, and emergency medicine.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
- Provide education and support to patients and their families regarding treatment options and resources available.
- Advocate for patients within the healthcare system to ensure they receive necessary services.
- Document all interactions and updates in patient records accurately and timely.
- Participate in quality improvement initiatives aimed at enhancing patient care delivery.
Requirements
- Current Registered Nurse (RN) license in good standing.
- Experience in behavioral health, pediatrics, or other relevant nursing specialties preferred.
- Strong clinical assessment skills with the ability to perform diagnostic evaluations.
- Excellent communication skills for effective interaction with patients, families, and healthcare professionals.
- Ability to work collaboratively within a multidisciplinary team environment.
- Knowledge of case management principles and practices is a plus.
