Overview
We are seeking a compassionate and dedicated Registered Nurse for our Home Health division. This role involves providing high-quality nursing care to patients in their homes, ensuring their comfort and well-being while promoting independence. Duties
- Assess patients' health status and develop individualized care plans based on their needs.
- Administer medications and treatments as prescribed, including acute pain management and sterile processing.
- Provide specialized care for patients with conditions such as dementia, pediatrics, and those requiring ventilatory support.
- Perform procedures such as spinal taps and botulinum toxin treatments when necessary.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Educate patients and their families about health management, medication administration, and self-care techniques.
- Monitor patient progress and adjust care plans as needed.
- Document all patient interactions accurately in compliance with healthcare regulations.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Experience in home health care or similar settings is preferred.
- Strong knowledge of MDS (Minimum Data Set) assessments and documentation.
- Familiarity with ambulatory surgery centers is a plus.
- Skills in dementia care, pediatrics, acute pain management, and ventilatory support are highly desirable.
- Ability to work independently while managing multiple patient cases effectively.
- Excellent interpersonal skills to build rapport with patients and families.
