Position Overview:
We are seeking a Lead Residential Counselor to provide person-centered services through the development and implementation of Individual Program Plans (IPPs) for the individuals we serve. The Lead Residential Counselor will play an essential role in assisting clients to maintain and/or increase their independence within their residences. You will collaborate with the Site Supervisor and the team to ensure that the preferences and interests of the persons served are honored and integrated into their daily lives.
Key Responsibilities:
- Person-Centered Services: Support clients in creating and maintaining individualized program plans that align with their goals and preferences. Focus on enhancing independence and life skills through tailored interventions.
- Assist with Independence: Help individuals maintain and/or increase their independence within their residence by providing the necessary support in daily living activities, medication management, and personal care as needed.
- Collaborative Support: Work closely with the Site Supervisor to ensure that the preferences, needs, and interests of each person served are reflected in their services. Provide input on the development and revision of care plans.
- Guidance & Mentorship: Provide leadership and support to other residential staff. Be a role model in carrying out tasks with professionalism, compassion, and consistency.
- Documentation & Reporting: Accurately complete documentation related to clients’ progress, activities, and incident reports, ensuring compliance with agency policies and regulatory standards.
- Crisis Management: Be prepared to intervene in any emergency or crisis situations, utilizing de-escalation techniques and ensuring the safety and well-being of the client and the team.
- Team Collaboration: Work as part of an interdisciplinary team, collaborating with therapists, case managers, and medical professionals to ensure holistic care for the residents.
Qualifications:
- High school diploma or equivalent required
- At least 1 year of experience working in a residential care setting, preferably with individuals who have Serious and Persistent Mental Illness (SPMI).
- Strong communication skills and the ability to engage with clients, families, and team members.
- Ability to work independently and as part of a team in a fast-paced environment.
- Excellent organizational skills and attention to detail.
- Ability to pass a background check and drug screening.
Preferred Qualifications:
- Bachelor’s degree in Psychology, Social Work, Healthcare Management, or a related field. Master’s degree preferred.
- Experience developing and implementing Person-Centered Plans (PCPs) or Individual Program Plans (IPPs).
- Knowledge of behavior management techniques and crisis intervention strategies.
- CPR, First Aid, and other relevant certifications (or willingness to obtain).
