Key Responsibilities
"• Develop Individualized Program Plans and Family Partnership Plans with participants/families
"• Provide case management services to families referred by Child Welfare Services (CWS)
"• Collaborate with community resources to provide support services to families
"• Conduct home visits and assessments to determine family needs and develop plans to address them
"• Monitor progress and make adjustments to plans as needed
"• Maintain accurate records and reports of family interactions and progress
"
Requirements
"• Bachelor's degree in social work, psychology or related field required
"• 2 years experience working with families who have experienced trauma and loss
"• Knowledge of child growth, development and behavior
"• Ability to work flexible hours, including evenings and weekends as required
"• Valid Hawaii driver's license and clear driver's abstract required
"• CPR and CPI training required within first 6 months of employment
" • Behavioral Management certification required within first year of employment
