**Responsibilities:**
- Assess patient needs and develop comprehensive care plans
- Collaborate with interdisciplinary teams to ensure seamless transitions of care
- Coordinate post-hospital care services, including home health, infusion services, and durable medical equipment
- Provide education to patients, families, and healthcare providers on issues related to utilization of resources and medical necessity
**Requirements:**
- RN diploma or associate degree in nursing
- Two years of medical/hospital nursing experience
- Knowledge of community resources and post-acute care programs across the continuum
- Familiarity with CMS Conditions of Participation for Discharge Planning
