This role involves working closely with healthcare professionals and patients, ensuring the most effective, safe, and economical patient-centered care is provided.
Responsibilities
As a Permanent Clinical Pharmacist, you will
- Provide comprehensive pharmaceutical care through integrated drug distribution, clinical services, and teaching.
- Collaborate with healthcare professionals to plan, monitor, review, and evaluate patient effectiveness.
- Recommend drug therapy changes when necessary and appropriate.
- Provide drug information and education to patients, healthcare professionals, and students.
- Develop, implement, and maintain processes and programs to ensure patient safety, improve patient outcomes, and reduce medication errors.
- Participate in the development and delivery of educational programs for the healthcare team, patients, and community.
The ideal candidate should possess the following qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- A minimum of 1 years of experience in clinical pharmacy, preferably in a hospital setting.
- License to practice pharmacy in any state.
- Certification in Basic Life Support (BLS) is required.
- Demonstrated ability to work independently and make decisions in complex clinical situations.
- Strong interpersonal skills with the ability to work collaboratively with other healthcare professionals.
