    Home Care Field / Visiting RN seeing patients in their homes. Territory is Manhattan

    What you will be doing :

  • Complete initial assessments, OASIS, recertifications
  • Creates and implements a plan of care
  • Collaborates with the interdisciplinary team and coordinates patient care
  • Educates patients and families

  • RN license in NY
  • Fluent in Spanish
