Job Details
The On Call Center Triage RN is responsible for the emergent triaging of calls received from patients and / or caretakers to for the purpose of telephonically assessing their needs in order to treat emergent situations that have developed during the On Call WEEKEND Center hours
Hospice Triage RN provides patient physical and psychosocial health systems assessments telephonically, supporting the Plan of Care, “Goals of Care” based on the patient’s and caretaker’s needs / wishes, and in coordination with plan developed / placed by the primary RN Case Manager for the patient.
