We are seeking a Nurse Practitioner for our Bakersfield location to join our interdisciplinary team and deliver patient-centered care to our senior participants.
Responsibilities
- Conduct initial and ongoing assessments of participants, including medical history, physical exams, and diagnostic evaluations.
- Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary team.
- Manage chronic illnesses and adjust medications as necessary to optimize patient outcomes.
- Provide preventative care, health education, and counseling to participants and their families.
- Coordinate referrals and consultations with medical specialists.
- Ensure continuity of care by monitoring participants’ progress and updating care plans as needed.
- Document care provided, ensuring timely and accurate medical records.
- Participate in team meetings to discuss care plans and strategies for patient well-being.
- Serve as a resource for the interdisciplinary team regarding geriatric care.
- Certified Nurse Practitioner (NP) licensed to practice in California.
- At least 1 year of experience working with an elderly or frail population preferred.
- Strong skills in patient assessment, chronic disease management, and geriatric care.
- CPR/BLS certification required.
- Up-to-date immunizations and medical clearance for communicable diseases.
