Job Details

What you will be doing
- **Patient Care Management** Provides direct medical care and symptom management for patients in hospice, ensuring comfort and quality of life. This includes administering medications, addressing pain or other distressing symptoms, and coordinating with other healthcare providers.
- **Family Support** Offers emotional support and guidance to patients' families, helping them understand the care plan, manage end-of-life issues, and cope with grief and caregiving challenges.
- **Emergency Response** Responds to urgent situations or crises that may arise outside of regular office hours, providing immediate care and deciding when to escalate issues or adjust care plans.
- **Documentation and Coordination** Maintains accurate and up-to-date patient records, communicates with the hospice care team, and ensures that care plans are followed and any changes are documented and communicated effectively.
- **Indiana RN license, or ability to obtain
