Job Summary :
- The role involves managing cases with complex social needs for long-term placements and providing treatment to adults, adolescents, and children.Key Responsibilities :
- Conduct initial interviews with members by eliciting and interpreting the psychological and social meaning of complaints.
- Determine urgency of members' needs.
- Provide a professional interpretation of members' presentations and recommend a course of therapeutic action, selecting appropriate treatment modalities.
- Render appropriate and immediate guidance and therapy for members in crisis or in need of clinical services.
- Maintain written records of therapy, prepare reports and letters; contact social agencies, schools, relatives, and friends (with member permission) to obtain pertinent information.
- Provide clinical consultation in an integrated health care environment regarding family interactions, sociocultural determinants, and community resources.
- Provide telephone assessment, counseling, resource information, and referrals.
- Demonstrate expertise in a variety of treatment methodologies.
- Provide appropriate and timely documentation of clinical encounters in members' charts.
- Adhere to all mandated guidelines for patients' clinical care and license.
- Maintain license.
- Participate in QM & CQI activities.Required Qualifications :
- LCSW with 2 years of experience.
- Knowledge and ability to identify community resources available.
- Experience managing cases with complex social needs for long-term placements.
- Competencies in timely and appropriate assessment, diagnosis, treatment planning, and treatment intervention of members.
- Effective communication of clinical material to members and clinical staff.
- High internal and external customer / patient satisfaction.
- Proficient in identifying and treating the span of developmental stages.
- Proficient in various treatment modalities including, but not limited to, individual, group, family, and child therapy.
- Knowledgeable of KP and community resources.
- Compliance with organizational, departmental, state / federal guidelines, and all regulatory agency policies and procedures.Certifications :
- BLS required.
- COVID Certificate with booster dose required. \n Certification : Basic Life Support
