Duties

- Conduct thorough documentation reviews to ensure compliance with regulatory requirements.
- Develop and implement policies and procedures that align with best practices in home care.
- Collaborate with interdisciplinary teams to create individualized care plans for patients.
- Ensure that all nursing staff are trained on MDS (Minimum Data Set) assessments and documentation.
- Monitor patient outcomes and adjust care strategies as necessary to improve quality of care.
- Maintain up-to-date knowledge of industry trends, regulations, and best practices in acute care.
Experience
- A valid nursing license with significant experience in home care settings is required.
- Proven experience in managing nursing teams and overseeing shift operations.
- Strong understanding of Medicare guidelines, MDS assessments, HIPAA compliance, and documentation standards.
- Excellent communication skills with the ability to lead, mentor, and motivate nursing staff effectively.
- Experience in developing care plans tailored to individual patient needs is highly desirable.
