Job Description:
We are seeking an experienced RN Hospice Case Manager to provide compassionate end-of-life care to patients in a home health setting. The ideal candidate will have strong case management experience and the ability to deliver high-quality care tailored to the needs of hospice patients and their families.
Key Responsibilities:
- Provide comprehensive hospice care to patients in their homes.
- Conduct patient assessments and develop individualized care plans.
- Coordinate with physicians, caregivers, and interdisciplinary teams to ensure optimal patient outcomes.
- Administer medications, symptom management, and pain control measures.
- Educate patients and families on hospice philosophy, care plans, and available resources.
- Maintain accurate and timely electronic documentation in compliance with hospice regulations.
- Provide emotional support and guidance to patients and families throughout the hospice process.
Qualifications & Requirements:
- Active RN license in Vermont – Required.
- Minimum of 2 years of hospice case management experience – Required.
- Experience providing hospice care in a home setting – Required.
- Familiarity with McKesson/Netsmart EMR – Preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and patient advocacy skills.
Shift & Schedule:
- Monday - Friday schedule (day shifts).
- 20-week assignment.
Healthcare Setting:
- Home health hospice care environment, working with patients in their private residences.
- Collaboration with physicians, social workers, therapists, and hospice support staff.
Physical Setting & Work Environment:
- Travel required within the Barre, VT area to provide in-home patient care.
- Work independently in a non-facility-based setting, ensuring timely and efficient patient visits.
- Provide hands-on care, requiring physical stamina, lifting, and mobility.
