About this position:
The High-Risk patient care and outreach nurse navigator provides complex obstetric patients with specialized education, patient advocacy, follow‐up, support, and coordination of care throughout the duration of their pregnancy and after discharge. This role will work closely with community Obstetricians and Perinatologists who are aligned with Sibley Memorial Hospital to connect with high-risk mothers early in their pregnancy and serve as a guide and resource for them through their care journey. The high-risk patient care and outreach nurse navigator will be the primary touchpoint for the patient while coordinating care with each of her supporting medical teams—Obstetrics, Maternal Fetal Medicine, Neonatology and any relevant subspecialties.

What you will need:
- Minimum nursing Baccalaureate degree (BSN) from an accredited school
- RN license in the District of Columbia
- Requires at least two years’ nursing experience in Women’s & Infants (W&I); Nurse Navigator experience may be considered in lieu of W&I experience.
- At least two years of inpatient acute care experience preferred
- Basic Life Support (BLS) required
Salary Range: Minimum 46.18 per hour - Maximum 71.58 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
