Summary:
The Case Management Coordinator shall respond to facility and community-based service requests from Case Management staff and is accountable for facilitating, implementing, and confirming post-acute care arrangements. The Coordinator will identify obstacles and delays to fulfilling requests and works collaboratively with Case Management to resolve. This individual will report internal or external quality issues or concerns to Manager or Director. The Coordinator shall document actions, interactions, and arrangements in the medical record. Additionally, the Coordinator will effectively work with Case Managers to facilitate Skilled Nursing Facility, hospice, acute rehab, LTAC, home health, and home medical equipment referrals and acts as a resource for information about these services. The Coordinator will work with Case Managers to disseminate IMM letters to appropriate patients. This individual shall schedule patient appointments such as therapy, wound care, PCP, etc, as well as to contact pharmacy to obtain medication pricing. The Coordinator will complete payer forms, transfer packets, non-clinical sections of wound vac forms, and others as directed by the Case Managers. The Coordinator shall arrange patient transportation, and contact homeless shelters regarding availability. The Coordinator shall verify insurance coverage and communicate with payers to relay necessary information for service authorization. The Coordinator is responsible for other duties as assigned by Manager.
Requirements:
Associate's degree preferred. Minimum one to three years healthcare experience. Proficient in the use of computer systems and software.
