Job Title :

Community Health Nurse RN

Job Specialty : Home Health Services

Job Duration : Full-time

Shift :

8-hour day shift, Monday to Friday, with some possible weekend visits

Guaranteed Hours : 40 hours per week

Experience :

Minimum of 1 year of experience in Home Health preferred, but training available for the right candidate

License :

Current Registered Nurse license in the state

Certifications :

Graduate from an accredited school of nursing

Must-Have :

  • Active Driver's License
  • Auto Insurance active and in good standing

Job Description :

  • Perform duties under the general medical direction of the Director of Home Health.
  • Participate in planning public health and education programs.
  • Conduct home visits, design patient treatment plans, and set patient care goals.
  • Serve as a liaison between patients and physicians.
  • Attend staff meetings.
  • Assist with weekend patient care coverage as designated by the Director.
  • Participate in public health screenings and follow up on clients or refer them to their physician as appropriate.
  • Provide health education to patients and caregivers.
  • Evaluate and assess individuals and families, considering physical, social, cultural, emotional, and environmental factors.
  • Interpret guidelines to determine the patient's payment mode.
  • Work collaboratively with the departmental Director to update care quality.
  • Accurately complete all required documentation for patients.
  • Work with area hospitals, nursing homes, and physicians regarding discharge planning as needed.
  • Stay current on changes to OASIS and Coding guidelines.
  • Perform any other duties deemed necessary by the Hospital Administrator.
