General Position Summary: The Licensed Practical Nurse (LPN) works in conjunction with a medical provider to provide professional, team based medical care and nursing support in a clinical setting. Under clinical supervision of the Lead Nurse, Unit Lead, Associate Director of Nursing (ADON), or provider and the Chief Nursing Officer (CNO), the nursing staff plays a critical role in the delivery of quality, patient-centered medical care within the context of a federally (HRSA) qualified community health center. Patient Relations
Essential Duties and Responsibilities:
- Communicates with patients in a courteous and professional manner in face-to-face encounters and via the telephone.
- Educates the patient and family members on the patient’s diagnoses’ and health care selfmanagement within scope of knowledge and authority.
- Participates with the provider in the implementation of the plan of care and multidisciplinary patient education.
- Promotes the Team-Based Care Model by participating in daily huddles and monthly care team meetings to communicate and collaborate on patient issues and needs.
- Conducts patient screening for hearing, vision, vital signs to include blood pressure, height, weight, and other routine screening procedures as needed.
- Assists the providers with in-office tests and procedures as ordered.
- Administers medications including intramuscular (IM), subcutaneous, and intradermal injections as ordered by the provider.
- Documents clinical activities and patient interactions in the medical record.
- Collects specimens from patients, performs in-house laboratory testing, and processes specimens and requisitions for transport to the outside testing facility.
- Stocks exam rooms and special treatment rooms with necessary supplies and indicates necessity for ordering supplies on the appropriate supply order form or on the designated board or list.
- Demonstrates enthusiasm and commitment toward continuous learning and developmental opportunities.
- Returns patient phone calls promptly.
- Demonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary.
- Functions as a triage nurse.
- Interacts with other agencies/medical entities as needed to obtain patient medical information, to ensure continuity of care, and to ensure patients receive necessary durable medical equipment (DME), prescriptions, and other supplies.
- Participates in education of nursing students, medical assistant students, and new hires as assigned.
- Other clinical duties and responsibilities as assigned by the Lead Nurse, Unit Lead, ADON, or CNO.
- Physical attendance is an essential element of the job and necessary to perform the essential functions of the LPN position.
- Attends and participates in JHC and other meetings as required.
- Will supervise nursing and support personnel as assigned.
- Attends all nursing department meetings and participates in the JHC Quality Improvement (QI) program.
- Attends all mandatory OSHA infection control trainings and follows general OSHA guidelines for infection control, including “Standard Precautions”.
- Abides by all JHC infection control policies, including acute response measures in the setting of a public health concern or disease outbreak.
- Performs skills evaluation a minimum of every two years utilizing the RN/LPN/Medical Assistant Competency Skills Form.
- Staff will abide by the Code of Conduct as documented in the Corporate Compliance Manual.

- Must be licensed with the Virginia State Board of Nursing with a current LPN license.
- Must have graduated from an accredited nursing program.
- Minimum 1-3 years’ experience as an LPN preferred.
