Job Summary:
- The primary function is to generate and manage leads from various referral sources while liaising between the Agency and the community regarding community and customer needs.
- Help identify new patients in need of home health services.
- Develops and Maintains relationships with payors and community referral sources/organizations to achieve referral and admission volume.
- Must grow and maintain a pipeline of patient referral sources to increase the agency's overall patient census consistently.
