Job Summary for a Home Health Registered Nurse:
Primary functions are to administer skilled nursing care for clients in their place of residence, coordinate care with the interdisciplinary team, patient/family, and referring agency; and assume the responsibility for coordination of care. Oversight of the Plan of Care, OASIS, Episode management with HCHB EMR. On-Call rotation must be willing to see patients if needed, training, skills check, etc. You will have the support of the Executive Director, and Clinical Manager in the parent office.
Essential Functions of a Registered Nurse:
- Coordinates services. Informs the physician and other personnel of changes in the patient's/client's condition and needs. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care. Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician.
- Monitors assigned cases to ensure compliance with requirements of third-party payors.
- Prepares clinical and progress notes. Completes appropriate documentation promptly.
- Demonstrates commitment and professional growth by participating in Agency-sponsored in-service programs and maintaining or improving competency.
- Participates in the Agency's QAPI program
- Promotes the Agency's philosophy and administrative policies.
- Provides on-call services to patients/clients and their families as assigned.
- Provides effective communication to patients/clients, their family members, team members, and other health care professionals.
- Job Qualifications of a Home Health Registered Nurse:

- Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Licensure: Current Texas State license as a Registered Nurse, Current Texas Driver's License
- Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
- Transportation: Reliable transportation. Valid and current auto liability insurance
