We are seeking a Dual Diagnosis Counselor for our treatment center in Willoughby, Ohio. In this role, you will provide therapeutic services for individuals with co-occurring mental health and substance use disorders.
Job description
As a counselor at Jordan CRC, you will be responsible for documenting clinical services within 24 hours of service delivery and ensuring all necessary documentation for timely billing is completed. You will manage the entire patient care cycle, including assessing, planning, implementing, monitoring, and evaluating actions to address clients' mental health needs.
Job Detail:
- Integrated treatment model
- 6 - 10 hours per week
- Monday – Friday / Days or Evenings (You decide your schedule. Must conduct a minimum of at least two consecutive 1-hour sessions per workday)
- Onsite or Telehealth (from anywhere that has a reliable internet connection)
Duties and Responsibilities
- Assessing clients using psychometric screeners or other evidence-based assessment tools (i.e., PHQ-9, GAD-7, etc.) and diagnosing clients for substance use and/or mental health disorders using the most updated Diagnostic Statistical Manual (DSM).
- Individual sessions with clients in accordance with state regulations and Joint Commission standards.
- Document clinical service delivery within 24 hours of seeing clients and according to state standards including with any internal or external person associated with the client.
- Documenting all counseling services, assessments, reassessments, referrals and follow-up in the client's medical record, and providing appropriate signatures and dating of such entries, including those made in electronic records.
- Collaborating with the client to develop a written treatment plan that is individualized, client-centered, and recovery oriented and includes goals and measurable objectives.
- Reviewing and, where necessary, revising the counseling portion of the client treatment plan to address emerging problems.
- Providing counseling services specified in the client's treatment plan.
- Reviewing clients throughout the treatment episode according to ASAM criteria, to determine the need for continued services or discharge/transfer.
- Developing the client discharge/transfer plans to ensure movement to the appropriate levels of care are successful.
- Participating as a member of a multidisciplinary team for assigned clients.
- Provides crisis intervention as needed.
- Demonstrating competency by chart reviews, quality assurance reviews, and supervisory reviews.
- Attend all staff trainings and Team meetings (via zoom).
- Performs other duties as assigned.
Compensation:
- Part-Time
- Contracted
- $28 - $45 per hour (depending on licensure and experience)
License Requirements:
- Licensed Social Worker – LSW, LISW - Licensed Professional Counselor – LPC, LPCC, LCPC
Additional Requirements
- Bachelor’s degree in social work, Psychology, Counseling, or a related field. Master’s degree preferred.
- Minimum of two years of experience in the provision of social services or mental health services.
- Provides Human Resources with copies of all continuing education certificates to facilitate compliance with state and/or federal regulatory boards.
- Three (3) employment references 2 Personal and 1 Professional.
- Cleared criminal background check and signed statement regarding felony indictments/convictions
- Cleared drug screen
