1.0 BASIC ROLE
1.1 Serve as the primary contact for unsheltered folks as they navigate systems related to obtaining permanent housing.
1.2 Provide Coordinated Entry Intake and Assessment Services for unsheltered folks, as well as other housing and service referral systems.
1.3 Identify unsheltered folks by contacting local support service providers, especially Rensselaer County Adult Protective, Health Home Care Coordination, Samaritan Case Management, local Detoxification Units and drug/alcohol Rehabilitation programs, Bethany Hospitality Center/Unity House, local meal programs, drop-in centers, Rensselaer County Jail, local law enforcement agencies, as well as other public and private service providers who have contact with unsheltered folks.
1.4 Maintain contact with unsheltered or Coordinated Entry-enrolled folks in local Mental Health Inpatient Units, local Hospital medical units, local detox and drug/alcohol rehabilitation programs, Rensselaer County Jail, food pantries, soup kitchens, and on the streets.
1.5 Provide follow up services to Coordinated Entry folks placed into Permanent Housing to help ensure housing stability and prevent re-entry to homelessness.
1.6 HMIS Data Entry; accurate and timely
1.7 Maintain a presence in Rensselaer County via foot and vehicle
1.8 Embrace and follow Harm Reduction strategies and Housing First principles
1.9 Per The Office of Mental Health; Provide initial and follow up support to individuals and families with histories of chronic homelessness and serious and persistent mental illness diagnosis and disabilities who are unsheltered. Identifies supports and assists homeless and chronically homeless adults and individuals living in places not intended for human habitation and have no permanent address or are living in substandard and unsafe housing in procuring safe and appropriate housing. Assist by providing referrals to behavioral health providers or medical providers, legal services, employment services and other community resources as needed. 2.0 KEY RESPONSILBILITES
2.1 Provide service triage and advocacy services to unsheltered folks and those enrolled as Coordinated Entry clients.
2.2 Provide advocacy to unsheltered folks currently enrolled in Joseph’s House & Shelter’s Code Blue Program.
2.3 Provide all necessary information into HMIS for all folks enrolled in the Coordinated Entry Advocacy Program, and to ensure that CE folks financial needs are met.
2.4 Complete, review, and maintain case records for all Coordinated Entry folks while providing one-on-one support, crisis intervention, and goal setting for Coordinated Entry clients.
2.5 Accompany Coordinated Entry folks to appointments as necessary.
2.6 Facilitate case management with other service providers to ensure that Coordinated Entry folks are served without duplication.
2.7 Complete, review, and maintain case records for all Coordinated Entry folks.
2.8 Provide one-on-one support, crisis intervention, and goal setting for Coordinated Entry folks
2.9 Conduct face-to-face interviews with persons to determine admission status and contract requirement
2.10 Build relationships and assess life circumstances, housing history, and income maintenance stability of identified unsheltered folks and Coordinated Entry folks through regular follow-up visits.
2.11 Attend all Rensselaer County Coordinated Entry Meetings
3.0 PERIPHERAL RESPONSILBILITES
3.1 Provide unsheltered folks with personal hygiene items, food, clothing and blankets.
3.2 Make sure unsheltered folks are familiar with local soup kitchens/food pantries and know when and where to access them.
4.0 STAFF/TEAM RESPONSIBILITIES
