Summary
As a MSW with Josephine at Home, you will play a vital role in ensuring the delivery of compassionate care to our clients. This position is essential in fostering a supportive environment that aligns with our mission and vision of being the community of choice for comprehensive caregiving.
Responsibilities
- Supervise and mentor social work treatment plans, providing guidance on case management and client interactions.
- Ensure compliance with HIPAA regulations and maintain confidentiality in all aspects of care.
- Conduct assessments and diagnostic evaluations to determine client needs and appropriate interventions.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams.
- Provide counseling services to clients and families addressing aging issues and other concerns.
- Monitor the effectiveness of care plans, making adjustments as necessary to improve outcomes.
- Facilitate group counseling sessions as needed to support residents' emotional well-being.
- Stay informed about community resources to assist clients in accessing additional support services.
Qualifications
- Master's degree in Social Work (MSW) with licensure required.
- Proven experience in case management within healthcare or social services settings.
- Strong understanding of aging issues and the ability to provide compassionate counseling.
- Knowledge of HIPAA regulations and medical terminology is essential.
- Excellent communication skills for effective interaction with clients, families, and staff members.
- Experience in supervising or leading a team is preferred but not mandatory.
- Ability to conduct diagnostic evaluations and develop comprehensive care plans.
- Active and unencumbered Washington State Driver's License and auto insurance.
