Responsibilities

- Assist with meal preparation and ensure dietary needs are met.
- Monitor patients' health and report any changes or concerns to healthcare professionals.
- Engage clients in social activities and community outings to promote socialization and inclusion.
- Support individuals with memory care needs through structured activities and routines.
- Facilitate communication between clients, families, and healthcare providers.
- Maintain a safe and clean living environment for clients.
- Document daily activities, progress notes, and any incidents as required.
- Provide emotional support and companionship to enhance clients' well-being.
Skills
- Experience in hospice care or assisted living environments is preferred.
- Knowledge of memory care techniques and patient monitoring practices.
- Strong interpersonal skills with the ability to build rapport with clients and their families.
- Ability to work effectively with individuals on the autism spectrum or with other developmental disabilities.
- Proficient in meal preparation and understanding of nutritional needs for various populations.
- Background in social work or related fields is advantageous but not mandatory.
- Excellent organizational skills and attention to detail are essential for maintaining accurate records and ensuring client safety.
