Job Overview
As a Contracted Palliative Care Nurse, you will provide compassionate, patient-centered palliative care services in home and facility settings. You will work closely with an interdisciplinary team, reporting directly to a Nurse Practitioner, and play a vital role in supporting patients and families in managing symptoms, understanding their care goals, and improving their quality of life.
This position is per-visit based, allowing flexibility in scheduling. It may lead to full-time employment as our company continues to grow.
Key Responsibilities
- Conduct home and facility-based palliative care visits to assess and manage symptoms for patients with serious illness.
- Collaborate with an interdisciplinary team, including nurse practitioners, physicians, and social workers, to develop and implement individualized care plans.
- Educate patients and families on disease processes, symptom management, medications, and advanced care planning.
- Submit availability (days and hours) to help coordinate patient visits.
- Document all care activities in the electronic health record (EHR) in a timely and accurate manner.
- Maintain communication with the care team and participate in team meetings as needed.
- Coordinate care transitions with hospice or other providers when necessary.
Qualifications
- Active RN license in the state of Colorado.
- Minimum 2 years of hospice or palliative care experience required.
- Strong assessment, symptom management, and patient education skills.
- Ability to work independently while being an integral part of the palliative care team.
- Excellent interpersonal and communication skills.
- Must have a reliable vehicle, valid driver’s license, and auto insurance.
- Liability insurance required.
- Proficiency with electronic health records (EHR) preferred.
