Job description
- Conducting assessments to identify problems, eligibility for assistance and need for services
- Developing a professional care plan from the assessment findings
- Performs visits and reports to supervising RN
- Supervises Home Health Aides in the patient's home
- Observes infection control practices and uses standard precautions
- Must possess excellent customer service and communication skills
- Thorough and timely documentation of initial and ongoing assessments and outcomes
Requirements:
- Valid NJ RN License
- 1 year of work experience in the field based eldercare/home care
- Excellent documentation skills, ability to meet deadlines
- Ability to understand directions, carry out instructions and maintain records
- Ability to function independently with minimal supervision
- Excellent organizational & time-management skill
- Good communication skills and knowledge of community resources
Experience:
