About the Role:
JourneyCare Home Health seeks an experienced Occupational Therapist to provide in-home therapy services within the Blue Island, IL territory.
As an OT, you will work closely with patients, families, and our interdisciplinary care team to enhance daily activities and improve quality of life.
This position offers one-on-one care in a patient's home setting, fostering independence and helping individuals regain skills necessary to lead fulfilling lives.
Key Responsibilities:
- Provides occupation therapy services according to a written physician's plan of care and as defined in the state Occupational Therapy Practice Act.
- Develops treatment programs and establishes goals for improved function.
- Communicates plans of care to referring physicians and other professionals as needed.
- Maintains appropriate clinical records, clinical notes, and reports any changes in patient condition.
Qualifications:
- Graduate of an occupational therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
- Registered with the National Registration Examination of the American Occupational Therapy Association.
- At least Two (2) years of clinical experience as an occupational therapist.
- Reliable transportation, current driver's license, and appropriate automobile insurance.
