JourneyCare Home Health - an Addus family company, is seeking a compassionate and experienced Occupational Therapist to provide in-home therapy services to patients. As an integral member of the interdisciplinary care team, you will work closely with patients, families, and healthcare professionals to enhance the patient's ability to perform daily activities and improve overall quality of life.
Key Responsibilities:
- Develop and implement personalized treatment plans tailored to meet each patient's unique needs and goals.
- Provide one-on-one care in a patient's home setting, fostering independence and helping individuals regain the skills necessary to lead fulfilling lives.
- Collaborate with the interdisciplinary care team to ensure coordinated, efficient care.
Requirements:
- Graduate of an occupational therapy curriculum accredited by the American Occupational Therapy Association.
- Registered with the National Registration Examination of the American Occupational Therapy Association.
- At least two years of clinical experience as an occupational therapist.
- Reliable transportation, current driver's license, and appropriate automobile insurance.
- Current CPR certification.
