Position Description
The Facility Based Coordinator is responsible for coordinating and facilitating services and supports in a facility-based setting where people supported acquire, retain and improve skills in the areas of self-care, sensory motor development, socialization, daily living skills, and communication in order to pursue and achieve his or her personal community or employment goals. The Facility Based Coordinator reports to the Day Program Supervisor.
Essential Functions:
- Design individualized programs for people supported to gain skills necessary to access their community or employment in an integrative way and to support them to discover and pursue their goals. This includes activities that will support people to learn new skills.
- Coordinate the teaching of and teach transferable skills for participation in employment or community integration. This includes completing daily living activities for/with people supported (bathing, dressing, toileting, eating, housekeeping, ambulating, and transferring, etc.); developing their own routines; and learning new skills personalized to each person
- Coordination and training of facility-based instructors on how to teach skills to people- supported
- Evaluate people supported rotationally every 6 months when due, to help them develop new goals and share with appropriate director
- Administer medications according to guidelines
- Counts medications according to guidelines
- Bend, stoop, push, pull, reach, lift up to 50 lbs.
- Treat persons- supported with respect at ALL times
- Coordinate the implementation of and follow various plans such as dining, therapies, etc.
- Encourage and coordinate community participation by educating, exposing, and providing experiences within the community
- Document observations and activities and person supported responses to activities
- Assist people with planning (menus, activities, outings, goals, etc.,)
- Assist the Program Supervisor with staff scheduling, activity scheduling, staff training, documentation and oversight as assigned.
- Support people (not do for them) with personal responsibilities
- Communicate and work with other team members effectively to aid in improving the quality of peoples lives
- Participate in team meetings for people supported
- Transport and accompany people supported on outings in the community to practice outcomes
- Maintain confidentiality of people supported
- Closely follows agency policies and procedures
- Complete accurate Reportable and non-reportable events before end of shift, as needed
- Meet attendance requirements at assigned location(s)
- Fill in for other staff when needed
Other Functions
- Attend training as assigned
- Complete emergency drills as assigned
- Participate in agency committees as assigned
- Perform job coaching duties as assigned
- Complete job contacts as assigned
- Cover shifts in different departments as needed
- All other duties as assigned by management
Required Skills
- Competent in CPR/First Aid
- Basic computer skills including typing (MS Word, Excel, Agency Software)
- Competent in written and verbal communication skills, which includes ability to effectively communicate training related information
- Strong attention to detail, analytical, and organizational planning skills
Work Environment/Conditions
The physical environment requires the employee to be able to sit, stand, and walk for periods of time, which vary. Additionally, must be able to move in such a way to assist supported persons ambulate, bath, dress, transfer, and toilet and lift up to 50 pounds. The working environment requires the ability to work flexible hours including nights, weekends, and holidays as needed. The Facility Based Coordinator is a full-time, salaried, non-exempt position based on a 40-hour work week.
Qualifications
- Must be 18 years of age or older
- High school diploma or equivalent
- 2 years of experience teaching skills
- Class D Driver license for State of Tennessee
Supervisory Responsibility
This position has NO supervisory responsibility.

- TB skin test or assessment within 30 days of employment
- Successful clearance of background check prior to employment
- Ability to pass any required security clearance prior to or post-employment
