Department: Dept. of Health - DOH - Duval County- Dentist.
Location: CHP Dental Centre, 515 W. 6th St. Jacksonville, FL 32206.
Schedule: Mon-Fri 8am - 5pm, 2-3 days per week
Interview Type: Webcam Interview Only.
Your candidate will have to pass a level 2 background and drug screen.
Must Be Fully Enrolled as a Florida Medicaid Provider
Work Environment: Dental Centre
Computer Software: EagleSoft Electronic Oral Health Record and HMS (Health Management System)
Dress Code: Medical Scrubs
IMPORTANT REQUIREMENTS:
* Must be a fully enrolled Florida Medicaid Provider
* Credentialed with DentaQuest Dental Insurance
* Credentialed with Liberty Dental Insurance
Candidate Responsibilities:
* Providing a full range of general dental services including comprehensive dental exams, preventive services, restorations, removable complete and partial dentures and extractions within the scope of a general dentist.
Experience/Skills Required:
* 1 year experience provide a full range of general dental services
Educational Requirement:
* DDS or DMD degree
* Active Florida Dental License.
Job Type: Contract
Pay: $95.00 - $100.00 per hour
