What are the responsibilities and job description for the Hospice LPN/RN for weekends position at JSSA?
Our Hospice team has immediate openings for part-time nurses available to work every other weekend to support our end-of-life patients and their families in Montgomery County.
Functions:
Deliver quality hospice and palliative care services to patients in their place of residence.
Make visits as assigned by the Clinical Manager to attend to patients need.
Serve as a clinical resource for referral sources and patients/family members.
Facilitate end of life conversations and decision-making with the family or caregiver.
Communicate with Clinical Manager, Doctors, and other clinical team members about patients' care as needed.
Chart all visits information in the EMR system.
Must be available to work every other weekend (Sat/Sun) from 8:30am- 5pm.
Position Qualifications:
An active State of Maryland LPN or RN license
Minimum 3 years of acute care, hospice and/or homecare experience.
Demonstrated knowledge and skills necessary to provide meaningful care and communicate with the terminally ill.
Excellent communication and customer service skills; attention to detail; and strong organization skills.
Highly proficient with MS Office 365, EMR/EHR systems and technology overall.
A valid driver’s license and reliable/independent transportation to travel across Montgomery County.
