Service Coordinator, Civil Action Project
Service Coordinator provides support to former justice-involved individuals looking to restore their civil liberties while removing barriers to employability, educational opportunities and housing.
Key Responsibilities
Receives new referrals from a variety of sources. Initiates assessment and interview process. Administers quick, high-risk assessment tool to identify a sub-group of participants needing intensive services.
Duties
· Data Entry
· Scheduling participants for fingerprinting, RAP sheet review, disposition requests with the court (as needed), and corrections to said requests
· Screening participants for eligibility and preparing: record sealing, Certificates of Relief, make referrals to LawNY/Public Defender’s Office
· Refer high-risk participants to LAEPEE program Case Manager, Intensive Services, and any other referrals provided in coordination with LAEPEE.
· Coordinate emergency needs such as food, bus passes, clothing vouchers, etc. for participants that follow through with their ELSAP program requirements.
Minimum Qualifications
Associates or Bachelor’s Degree in Human Services or Criminal Justice Studies preferred
