Job Description

Job Title : Clinical Social Worker

Job Specialty : Clinical Social Worker, Social Worker-Clinical

Job Duration : 13 weeks

Shift : Day - 8x5 - 09 AM

Guaranteed Hours : 40 hours per week

Experience : Minimum of 2-4 years in an inpatient or outpatient healthcare environment in social work

License : Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) preferred

Certifications : Valid State of Alaska Driver's License

Must-Have :

Ability to perform mental health evaluations
Skilled in treatment plan preparation
Experience in crisis counseling
Exceptional documentation skills
Competence in electronic health records and technology
Alaska licensure clearance

Job Description :

Work directly with patients and their families, providing support and guidance for financial and housing needs

Collaborate with state and federal agencies for placement needs and identify custodial issues

Refer patients in crisis to behavioral health services and handle referrals and consults from primary care and outlying clinics

Provide encouragement, coaching, and linkage with community services to support each patient's unique healthcare needs

Interview patients or their designees to obtain relevant psycho-social information and assess the impact of illness on the family

Participate in the development of an interdisciplinary plan of care for patients / residents

Coordinate the discharge planning process and make referrals for appropriate home or social services care and equipment

Offer therapeutic support and education to patients / residents and families

Assist patients / residents and families in understanding medical recommendations and make professional support referrals as needed

Participate in interdisciplinary team meetings and patient care conferences

Manage assigned caseload and document results of psycho-social assessments and plans in medical records

Maintain documentation that is timely and meets regulatory compliance and departmental standards

Engage in other duties to support the organization's vision, mission, and values
