JOB SUMMARY: The Nurse Case Manager supports the delivery of integrated primary care and HIV prevention services using a team-based approach. All Juniper Health employees will adhere to the Patient Center Medical Home (PCMH) model and work to ensure that patients of all ages receive comprehensive, coordinated and patient-centered “whole patient” care. The employee will maintain confidentiality at all times.
Qualifications
ESSENTIAL FUNCTIONS:
- Work closely with patients at risk of infection from HIV or living with HIV/AIDS.
- Coordinate HIV testing and/or follow up testing.
- Coordinate PrEP administration or linkage to appropriate HIV care and treatment within 30 days of diagnosis, as necessary.
- Advocate for the patient.
- Assist with housing, financial, and social support.
- Coordinate collaborative efforts.
- Schedule and participate in regular meetings, either in-person (following COVID-19 guidelines) or virtually.
- Develop intervention plan to reengage the patient if meetings are not maintained.
- Document treatment adherence activities, any education/counseling received, medical appointments attended, follow up testing performed, etc.
- Works with highly complex patients or patients with uncontrolled chronic conditions
- Works to the degree of their license in a clinical setting
- Other duties as assigned.
REQUIREMENTS:
- Graduate from an accredited nurse training program approved by the Kentucky Board of Nursing
- Successful completion of the National Certification Examination.
- Possession of a clean and current license to practice professional nursing in the Commonwealth of Kentucky.
- Must maintain current ACLS certification as well as continuing education credits as deemed necessary for licensure by the Kentucky Board of Nursing.
WORK ENVIRONMENT:
- Work is typically performed in a clinical office environment.
PHYSICAL DEMANDS:
- The physical demands required of the position are representative of those required of comparable professional positions and must be met by the employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee is required to communicate, operate equipment, observe details at a close range, be in stationary or moving positions, and occasionally lift or move up to 25 pounds. TRAVEL:
- Travel to JHI sites and/or external meetings and trainings may be required.
OTHER:
- The employee is expected to work and be available during regular business hours, however, extended hours and weekend work may be required.
