Who We Are Looking For:

We are looking for a Program Manager to join the team at our Assisted Living Program. The Assisted Living Program (ALP) helps people disabled due to HIV/AIDS who are homeless. ALP helps individuals obtain a housing subsidy, secure an apartment and then provides ongoing case management to help each individual maintain their health, housing and independence.
What You'll Do:
- Oversee case management services provided to a statewide subsidy and services program for persons living with HIV/AIDS and other disabilities to ensure a high quality of professional case management is being provided.
- Ensure that all program clients are connected to housing, primary care, HIV care, substance use and mental health treatment as needed, and mainstream benefits.
- Meet with individual case managers regularly to assess performance and provide support. Maintain clear and accurate supervision files for each staff person.
- Communicate on behalf of the program with outside contacts, including partner agencies, landlords, and clients, to troubleshoot issues as they arise.
- Serve as head of hiring team to recruit and hire case management staff. Orient, train and evaluate case managers.
- Facilitate regular team meetings.
- Safeguard the confidentiality of all staff and client records.
- Work closely with regional housing agencies acting in partnership with the program, to ensure efficient processing of applicants and appropriate service provision to housed clients.
- Work as part of JRI Health Housing/Case Management team including Program Director Housing Options Program Coordinator, and Clinical supervisor.
- Participate in management level meetings with other JRI Health Program Directors and Managers.
- Ensure case managers participate in regularly scheduled Group Clinical Supervision.
- Oversee case management work schedule, including ensuring caseload coverage during case manager vacations/extended time off.
- Ensure required reports are produced by Case Management team in a timely fashion to assist Database Administrator and Director with reporting requirements.
- Work with management team to evaluate overall program and assist with production of reports to funders and for internal review.
- Represent program at meetings with funders and at conferences/workshops.
- In the absence of the Program Director, assume extra duties as assigned by Executive Director.

Requirements:
- Must be 21 years or older.
- Bachelor's degree in human services, social work or related field desirable.
- Other candidates considered with an additional five years of human services experience.
- Experience hiring, orienting, training and supervising staff.
- Experience providing case management necessary, with HIV-specific experience preferred.
- Demonstrated knowledge of and sensitivity to issues related to HIV, substance abuse, poverty & homelessness and diverse populations required.
- Knowledge of federal housing subsidy system preferred.
- Self-directed, team oriented and flexible. Excellent oral and written communication skills.
- Experience using word processing software necessary. Ability to work effectively within strict deadlines.
- Bilingual/bicultural (Spanish or Haitian Creole) strongly preferred.
- A commitment to health equity including proven ability to work with populations disproportionately impacted by social and racial injustice including communities of color, LGBTQIA individuals, active substance users, homeless individuals, linguistic minorities.
- Candidates must have a valid driver’s license and access to reliable transportation.
- Background and driving record checks will be performed.
