JusticeWorks YouthCare is seeking a Family Finding Coordinator / Family Intervention Case Manager / FGDM Coordinator! In this position, you will have the opportunity to use your passion for helping children and families to make a lasting impact. The Family Finding Coordinator / Family Intervention Case Manager will be responsible for connecting children with family members who can provide them with stability and support.
You will also support families in our community by providing them with resources, advice, and assistance. You will be responsible for connecting families with local services, such as housing assistance, healthcare, and other support services. As a Family Intervention Case Manager, you will also provide guidance and direction to parents and families in need, helping them find the resources they need to create a better future for their children.
In addition, you will be responsible for coordinating meetings to facilitate family-centered decision-making processes for families with a child in out-of-home care. You will work collaboratively with families, caregivers, foster parents, court-appointed attorneys and other community professionals to develop and implement plans for the safety and well-being of the child. You will also provide education and resources to families and community partners. The qualified Family Finding Coordinator / Family Intervention Case Manager / FGDM Coordinator will:
- Discover, identify, locate and track relatives, kin or significant individuals for the referred child’s or family’s life.
- Engage the identified relatives, kin or significant individuals using a strengh based approach in order to develop a supportive network of connections for the youth or family.
- Make all necessary arrangements and follow procedures for alignment and decision making meetings (Issue invitations and communicate all relevant details about the alignment or decision making meeting to each participant).
- Assess and engage families in service planning based on demonstrated needs.
- Plan and implement appropriate interventions using evidence based and best practice approaches.
- Issue invitations and communicate all relevant details about the conference to the identified family members and support persons.
- Facilitate and/or co-facilitate meetings as needed.
- Ensure that all relevant steps in setting up the conference, carrying it out and following up actually occur.
- Engage families in service and treatment planning.
- Transport clients utilizing personal vehicle, as necessary.
