Care Team Specialist Crisis Prevention- Crocker

Position Purpose:

Under the direction of the Program Director, the Care Team Specialist is an integral member of a multidisciplinary team, dedicated to fostering engagement, retention, safety, and support for participants of the Crocker Care Campus. This role actively builds rapport with individuals, creating a sense of community and trust while promoting harm reduction, trauma-informed care, and participant autonomy. 

The Crocker Care Campus provides a welcoming, low-barrier space for people experiencing homelessness. Many participants also navigate challenges related to substance use, mental health, and systemic inequities, which may manifest in behaviors such as verbal outbursts or expressions of frustration. The program operates with a harm reduction model, embracing a "do whatever it takes" philosophy to support participant success while maintaining a safe and compassionate environment.

Principal Responsibilities:

  1. Establish a positive rapport with participants, providing a safe and supportive environment.
  2. Apply harm reduction and trauma-informed care principles in all interactions.
  3. Monitor common areas and building perimeter to ensure participant safety and security.
  4. Provide crisis intervention and de-escalation support using nonviolent techniques.
  5. Maintain sign-in/out logs for participants and visitors at the front desk.
  6. Conduct belongings searches and room checks as needed to uphold safety protocols.
  7. Serve as floor lead during fire drills, evacuations, and emergency situations.
  8. Complete detailed reports on incidents, participant concerns, and program operations.
  9. Assist with participant intake, engagement, and connection to supportive services.
  10. Facilitate communication between participants and care team members.
  11. Respond to participant concerns with empathy, de-escalation strategies, and problem-solving techniques.
  12. Ensure program areas remain clean, organized, and welcoming.
  13. Collaborate with emergency responders, behavioral health teams, and harm reduction specialists as needed.
  14. Maintain strict confidentiality and adhere to all organizational policies.
  15. Utilize AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) principles in all participant and team interactions.
  16. Other duties as assigned.

Physical Demands:

  1. Stand or walk for extended periods on various surfaces (tile, concrete, carpet).
  2. Climb stairs, ramps, or ladders as needed.
  3. Kneel, squat, bend, and run if necessary.
  4. Lift or carry up to 50- 70 lbs. occasionally.
  5. Use hands, arms, and fingers for reaching, handling, and grasping objects.

Requirements:

  1. High school diploma or equivalent 
  2. Experience working with individuals experiencing homelessness.
  3. Strong communication skills, particularly in de-escalation and crisis intervention.
  4. Ability to work with individuals actively using substances in a nonjudgmental, harm reduction-focused manner.
  5. Ability to work in a non-traditional clinical setting (e.g., outdoor park-lets, drop-in spaces).
  6. Demonstrated knowledge of harm reduction, trauma-informed care, and housing-first principles.
  7. Proficiency in basic computer skills (email, data entry, Microsoft Word/Excel).
  8. Ability to work flexible hours, including evenings and weekends.
  9. Bilingual (English/Spanish) preferred.
