What are the responsibilities and job description for the Community Health Worker position at JWCH Institute?
Position Purpose: Under the direction of the Program Manager or designee, the Community Health Worker will support the implementation and achievement of the goals for the Ending the HIV Epidemic Intervention Program targeting Black/African American and Latinx cisgender women with HIV. This will be accomplished by conducting outreach and presentations to promote the program, recruiting participants, assisting with the facilitation of group sessions, providing referrals to medical and support services within the agency, and supporting data collection and program reporting.
Principal Responsibilities:
- Actively seek opportunities for outreach, focusing on identifying hard-to-reach populations.
- Perform outreach activities to recruit clients for participation in the EHE Intervention Program.
- Develop rapport and establish trusting relationships with clients during Seeking Safety sessions, reinforcing positive lifestyles through follow-up engagements.
- Engage with the homeless population to encourage participation and promote the reinforcement of learned skills.
- Assist in gathering and compiling program data for the Project Coordinator/Manager and ASW.
- Provide support to participants, helping them build emotional and social support systems that motivate them to keep scheduled appointments, remain in care, and achieve viral load suppression.
- Participate in JWCH, DPH, and other relevant trainings as scheduled.
- Perform other duties as assigned.
- Attend JWCH trainings and advisory committee meetings as required.

Requirements:
- Minimum of 1 year of experience in outreach, case management, or providing health-related services to diverse populations, particularly Black/African American and Latinx communities.
- Experience working with individuals living with HIV or other chronic conditions is highly desirable.
- Valid driver’s license, reliable transportation, and proof of insurance.
- Strong ability to develop positive relationships with clients living in temporary housing (e.g. shelter; mission; hotel or substance abuse recovery facility).
- Excellent verbal communication skills.
- Ability to work with homeless; mentally ill and/or substance-abusing adults as well as professional staff.
Preferred:
- Familiarity and/or experience with Seeking Safety Intervention
- Experience working with the homeless population.
- Bilingual Capabilities (Spanish/English)
