Position Purpose: Ignite your career with passion and purpose with a remote tele psych, full-time or part-time options or contract basis as a Licensed Marriage and Family Therapist (LMFT) at JWCH! Immerse yourself in the heart of Southern California, covering one or more of our Primary Care Clinics, Residential and Out-Patient Substance Use Programs. As an LMFT, you'll be at the forefront of providing transformative behavioral health services, leading crisis intervention, delivering both brief and long-term counseling, providing case management, triage and linkages to social services and housing resources. Your role is important in ensuring comprehensive services that guarantee not just better health outcomes but also improved educational outcomes for our community.

Key Responsibilities:

  1. Work at a Federally Qualified Health Center multidisciplinary team to ensure that the patient’s clinical treatment goals and behavioral management needs are met by attending case conferences, chronic disease management meetings, staff consultation, and individual meetings with staff to ensure quality of care.
  2. Conduct functional and strength-based assessment along with comprehensive diagnosis for mental health, substance abuse, trauma, and psychosocial needs.
  3. Develop care plans that are coordinated and integrated with a comprehensive range of medical and psychosocial services to address social, emotional, and practical needs.
  4. Provide in-person and remotely (via HIPAA Compliant software) psychotherapy, counseling,  and psycho-education utilizing individual, family, and group modalities using evidence-based interventions for patients and their significant others that include social, emotional, and practical life skills to facilitate safe coping, distress tolerance, stress reduction, mindfulness, and relaxation as well as access to necessary resources with the goal of improved functioning and positive health outcomes.
  5. Utilize both short-term and intensive longer-term interventions, as approved by funding source that target trauma, mental health symptoms, increase psychosocial functioning and reduction of substance use.
  6. Interventions are client centered; strength based, and identified based on a comprehensive treatment plan that is developed, after completing a thorough biopsychosocial assessment.
  7. Consult with primary care providers to enhance understanding of the patient, provide support for treatment approaches that incorporate behavioral health needs, and assist with implementation of bio-psychosocial treatment and care plans.
  8. Work with Case Management staff to coordinate care and provide referrals, coordinate services and reduce barriers.
  9. Participates in weekly case conferences to assist in problem solving related to patients and to ensure that professional guidance and high-quality case management services are being provided.
  10. Participates in community meetings pertaining to the needs of JWCH patients.
  11. Participate in Multi-Disciplinary Team meetings that are held weekly in the clinics.
  12. Be available as assigned to provide after-hours emergency psychiatric assistance.
  13. Provide phone consultation as needed
  14. Complete all charting as required by JWCH, including assessments, progress notes, care plans, and billing documentation. Encounters must be closed by the end of each shift.
  15. Assist with provider education for integrated behavioral and primary care services
  16. Participate in behavioral health quality assurance/improvement activities to maintain and strengthen standards of care, in accordance with the organization CQI Plan
  17. Other duties as assigned

Requirements:

  1. Master’s degree in psychology from an accredited college or university.
  2. LMFT with current California BBS license
  3.  Demonstrated understanding of the impact of trauma on behavioral and medical conditions
  4. Knowledgeable of evidence-based treatments (sand and play therapy, DBT, CBT, Seeking Safety, Motivational Interviewing)
  5. Knowledge of principles, procedures, methods and techniques of mental health emergency procedures such as: 5150, CPS reporting, APS reporting, and Tarasoff reporting
  6. Experience working with substance abuse disorders preferred.
  7. Bilingual (English/Spanish) ability needed.

Note: If you become a JWCH provider, you may be able to participate in Loan Repayment Program. A majority of our clinics are NHSC approved sites. The National Health Service Corp may be accepting applications Loan Repayment Program awards. The program is open to licensed primary care medical, dental, and mental and behavior health providers who are employed or have accepted an offer of employment at an NHSC-approved site. Awardees receive up to $50,000 in exchange for two years of service if they practice in a HPSA of 14 above (Tier I). Awardees can also receive up to $30,0000 in exchange for two years of service if they practice in a HPSA of 13 and below (Tier II). You will need to contact National Health Service Corp to apply and receive more information. Upon acceptance, you are encouraged to apply to National Health Service Corp for Loan Repayment Program.
