What are the responsibilities and job description for the Nurse- Quality Improvement Analyst position at JWCH Institute?
Location: Based off Corporate office in Commerce, CA.
Classification: Full-Time, salary is based on qualifications, licensure and experience. (within range)

Position Purpose:
The Accelerating Cancer Screening (AxCS) Nurse QI Analyst will provide operational and quality oversight of the HRSA Accelerating Cancer Screening grant. The position will collaborate with key stakeholders, including the City of Hope, JWCH internal providers and support staff, patient and peer navigators, and various community partners, to operationalize outreach, provide patient education, and increase access to cancer screening. The Nurse QI Analyst will also be responsible for leading quality and process improvement projects including tracking screening rates across all JWCH clinics, generating provider care team reports, and monitoring referral arrangements for treatment.
Principal Responsibilities:
Outreach:
- Work with the JWCH Director of Quality Assurance to stratify patients according to screening needs for breast, cervical, and colorectal cancers and ensure screening communication is sent through text massages (Well-App), email, or mail that are created and sent to the appropriate patients and screening populations.
- Work with the JWCH Director of Development, City of Hope, and the American Cancer Society to create evidence-based and culturally tailored patient education and materials, including personalized messages, social media campaigns, flyers, posters, monthly patient health newsletters, and other media pamphlets to help inform and motivate patients to schedule and complete screening tests as part of the Cancer Screening for Life campaign.
- Schedule community outreach and awareness events with community partners, the City of Hope, JWCH Director of Development, and AxCS Peer Navigators.
- Work as the liaison between all key stakeholders and JWCH staff.
Quality Improvement:
- Generate provider cancer care team reports based on patient needs for screening and patients who are high-risk. Work with JWCH Patient Navigators to ensure that outreach is conducted at the clinic and care-team level to these patients.
- Work with JWHC Patient Navigators and provide oversight of the scheduling of patients for screening with the City of Hope mobile unit, Alinea, UMI, and other outside screening agencies.
- Work with the IT department to create a system of flagging and monitoring positive cancer screenings and create a registry of patients that will be monitored by the AxCS QI Nurse Analyst and the QI Department Patient Navigators
- Monitor diagnostic testing and specialty referrals of patients who have had abnormal screening tests and ensure that those patients who screen positive receive further diagnostic workup.
- Monitor referrals of patients with confirmed cancers who need evaluation with cancer specialists.
- Work alongside the AxCS Patient Navigators to coordinate the care of patients with positive cancer screens, including the rapid access and scheduling of further diagnostic testing and consultation with specialty care, when indicated.
- Track cancer screening rates at the agency and individual clinic level and create a dashboard that is updated monthly. This dashboard should also be available at the agency, clinic, and individual provider level, when requested.
- Coordinate process improvement projects, through use of PDSA cycles, to identify areas of improvement and increase cancer screening rates.
- Assume responsibility for completing and submitting quarterly grant reports to HRSA in a timely manner.
Requirements:
- Current and active licensure as a licensed healthcare professional (Licensed Vocational Nurse, Registered Nurse, Nurse Practitioner)
- Experience in adult health care, outpatient environment
- Current CPR and BLS certification
- High level computer and IT proficiency in the following software programs: Microsoft Word, Excel, and PowerPoint.
- Experience utilizing an Electronic Health Record system.
- Ability to work independently with minimal direction or supervision.
- Ability to write notes, records, and reports.
- Ability to work with patients of diverse social and economic backgrounds.
- Excellent organizational skills
- Ability to handle confidential information appropriately.
- Ability to communicate effectively with all levels of employees and outside contacts.
Preferred:
- Bilingual (Spanish/English)
- QI/QA experience
- Experience working with population health programs (e.g., i2i Tracks)
