Position Purpose:

Under the direct supervision of the Chief of Behavioral Health, the ECM/CS Program Director oversees and manages program operations, including hiring, training, and supervising a staff of 30 or more. This position is responsible for all aspects of reporting required by health plans, fiscal management, and all other phases of the project to provide enhanced care coordination and outreach services to individuals eligible for the Enhanced Care Management (ECM) Program or Community Support (CS) Services.

We operate over 16 ECM sites across the Los Angeles region and the Antelope Valley. This position will be based at our corporate office and will require regular visits to all ECM sites.

Principal Duties:

  1. Oversees the ECM/CS program and provides guidance and supervision to staff.
  2. In collaboration with clinical staff, interviews and hires qualified candidates for case manager, housing navigator, and administrative support roles.
  3. Develops and implements initial and ongoing staff training programs and schedules regular team meetings.
  4. Ensures that staff coordinate and schedule care conferences with PCPs, LCSWs, and other relevant professionals.
  5. Oversees program monitoring and ensures all reports and claims are accurate and submitted on time.
  6. Assists with reporting and makes improvements to ECM program workflows.
  7. Provides supervision for all Case Managers, Housing Navigators, and Patient Navigators under ECM, in coordination with Clinical Consultants.
  8. Conducts detailed orientation and training for all new ECM staff and ensures all staff participate in regularly scheduled Health Plan webinars.
  9. Ensures monthly staff meetings and training sessions are held to cover policies, procedural changes, and educational topics relevant to staff responsibilities.
  10. Collaborates with local homeless service providers and hospitals to deliver outreach and services to individuals eligible for program assistance.
  11. Ensures adherence to all state, county, and federal standards and regulations.
  12. Monitors the quality of care and conducts documentation reviews to improve processes.
  13. Submits monthly reports, including patient encounter data as required by Health Plans, along with any other necessary reports.
  14. Coordinates with the Billing Department and Health Plans to manage ECM billing processes.
  15. Interfaces cooperatively with other treatment agencies, primary care providers, and staff.
  16. Ensures staff thoroughly document assessments of care levels and treatment needs, client motivation, barriers to care access, treatment plans, case management progress, and other relevant details.
  17. Emphasizes a professional and compassionate relationship between clients and the HHP team.
  18. Performs other duties as assigned.

Requirements:

  1. Master’s Degree in Social Work with at least five years of experience in a case management setting post-graduation.
  2. LCSW preferred but not required.
  3. At least two years of experience managing and supervising another ECM program in LA County or a similar case management program in a community clinic setting.
  4. Experience working with homeless populations, co-morbid health conditions, substance use disorders, and chronic health and mental health issues.
  5. Experience in outreach and treatment programs within a community clinic setting.
  6. Excellent computer skills, including proficiency in working with EMRs, Excel, and various databases used by health plans.
  7. A valid California Driver’s License and an reliable transportation, as this position requires frequent travel between other JWCH sites (mileage is reimbursed).
  8. Excellent oral and written communication skills.
  9. Ability to work independently with a proactive approach and take initiative.
  10. Willingness and ability to work effectively with low-income, trauma-affected, and homeless individuals.
