Licensed Individual Clinical Social Worker (LICSW)

Job Summary

· The Social Worker is in charge for the assessment, planning, implementation, coordination, monitoring and evaluation of the patient’s plan of care from admission to post-discharge.

· The Social Worker utilizes critical thinking skills, and the principles of case management to coordinate and facilitate individual, group and family therapy sessions with the patients.

· Utilize consultation data and social work experience to plan coordinate patient care and following through to ensure service efficacy.

· Organize support groups or counsel family members to assist them in understanding, dealing with and supporting the patient.

Qualifications

· Current Licensure in Social Worker (LCSW/LICSW) in the District of Columbia.

· Master's degree in Social Work

· Bachelor’s Degree in Social work or closely related field such as Psychology, Child Development or sociology.

· Three years’ experience in acute care or other related healthcare field is preferred for LISCW/LCSW.

· Case Management Certification will be a plus.

· At least one year experience working with persons diagnosed &/or dual diagnosed with psychiatric illness & receiving active treatment.

· At least one year experience working as part of a multidisciplinary mental health team conducting individual, group and family therapy.

· Knowledge of ICAMS and/or Credible will be a plus.
