JOB SUMMARY
We are seeking a temporary employee with the possibility of becoming a permanent member of the team after a successful evaluation period. Responsible for all areas of client care; intakes, clients, scheduling, and oversee direct care staff .
JOB PURPOSE
The Service Coordinator has a variety of roles and responsibilities that must be fulfilled to ensure the smooth functioning of client care. Oversee the team responsible for coordinating and delivering care to clients to support their care as part of programs authorized by their health insurance provider. In this role, you will oversee direct care staff. The ideal candidate will drive the team to maximize the delivery of client care. This role ensures that our clients receive the care they need delivered in a timely and efficient manner.
You will be part of the Case Management Team that reports directly to the Social Worker / Administrator Assistant. As part of the team, the Service Coordinator will be involved in key initiatives and ensuring those initiatives are aligned with the business objectives.
The Service Coordinator will speak to prospective clients to educate them on our services, conduct in-home assessments with disabled adults/seniors, pediatric clients, and/or their families to sign them on for services, perform ongoing care management & communication with our clients for assured high quality services, represent our local office at community events, uphold our company values and high standards of care and make sure that our team of direct care staff, clients and referral-based contacts are all satisfied with our services.
JOB DUTIES AND RESPONSIBILITIES
- Case management liaison
- Manage Client Services caseload while proactively growing it.
- Proactively identify operational challenges and gaps and escalate and provide / maintain visibility to senior leadership to help ensure gaps and challenges are addressed.
- Providing after-hours support for our clients and prospective direct care staff (including evenings and weekends) – this support may include e-mail, telephone and/or home visits.
- Responsible to assure excellent customer service to clients, referral partners and others.
- Answering calls from referral sources, prospective clients and/or their family members to educate them about our services and care options and existing clients and conducting follow-up and other client development activities to generate new clients within the service area.
- Respond to client and/or caregivers requesting changes in schedule based on the current day visit hours.
- Enter and change schedules with approval of the client and/or family, inform all changes.
- Monitor and respond to Family Messages in regard to schedules and caregiver change request.
- Submit authorization requests to Medicaid insurance plans for services.
- Oversees direct care staff, including hiring, scheduling and assigning work, reviewing performance, demotions, or terminations. Trains, provides work direction, and problem-solving assistance for direct care workers.
- After adding a schedule, confirm acknowledgment of care plan and encourage them to ask questions. Inform shift starts.
- Complete Intakes and Appeals.
- Monitor incoming/outgoing facsimiles
- Coordinates and confirms schedule with clients and active caregivers.
- Assist patients and caregiver matching
- Other duties assigned

REQUIRED
- Operations management background, preferred.
- Excellent time management skills with ability to work independently and multitask effectively while maintaining attention to detail.
- Position requires proficiency in word processing and computer skills (Office, Excel, Outlook & Power Point). Will need to master additional software including EVV System.
- Must possess above average human relations, customer service, and organizational skills. Must be able to work under time pressures and manage multiple demands simultaneously, delegate when appropriate. Extreme attention to details is essential. Excellent telephone and communication skills are necessary. Must be a team player willing to share information and work cooperatively with other members of the work team and caregiving staff.
- Must be a problem-solver who always carry a smile on their face and compassion in their hearts. In addition, someone who is hard-working, ambitious, has a positive attitude, friendly, outgoing and professional.
- Experience in health care or home health care settings
- Strong understanding of health care systems and policies.
- Ability to work independently and manage a caseload
- Associates or Bachelor's degree in Human Relations or related field, preferred
- 2 years of experience in home health care or related field.
- Excellent communication verbally, written and interpersonal skills.
- Must be highly organized, detail oriented, and have exceptional planning.
- Strong organizational and problem-solving skills.
