We are a home and community-based Medicaid waiver provider serving the state of Florida. Supported Living Coaches (SLCs) are responsible for providing training and assistance to individuals with developmental disabilities, helping them achieve their goals of living as independently as possible in their own home or apartment. SLCs customize their teaching methods and strategies to suit the individual's abilities, learning needs, and preferences. SLCs may offer support in :

Finding and Securing a Home : SLCs may assist individuals in finding suitable housing options based on their preferences and needs. They may help with researching available housing, arranging viewings, understanding lease agreements, and transitioning into a new home.
Home Maintenance : SLCs may provide training and support to individuals in maintaining a clean and organized living environment. This may include teaching basic housekeeping skills, such as cleaning, laundry, and organization to ensure the individual can maintain their home independently.
Budgeting and Financial Management : SLCs may help individuals develop and maintain a budget, manage bills, and understand financial responsibilities. They may provide guidance on tracking expenses, setting financial goals, and accessing community resources for financial assistance if needed.
Meal Planning and Nutrition : SLCs may support individuals in planning healthy meals and developing cooking skills. They may teach the basics of meal planning, grocery shopping, and food preparation, based on the individual's dietary needs and preferences.
Community Integration : SLCs may help individuals connect with their local communities and participate in social activities. They may assist in finding recreational programs, community events, and other opportunities for engagement and socialization.

Description :

Ensure compliance with all policies and procedures, requirements of APD and other regulatory agencies.

Communicate and Coordinate with Personal Support services worker, where applicable.

Document services provided accurately and in a timely manner.

Maintain and update all required documentation in assigned case files in a timely manner.

Develop implementation plans with individual's input and provide needed support to each individual to achieve stated goals.

Maintain regular direct / in person contact with assigned individuals.

Provide support to assist individuals to achieve stated goals and outcomes.

Provide back-up Supported Living Coaching as assigned.

Ensure satisfaction and choice surveys are completed for each individual on an annual basis.

Assist the individuals to develop and maintain appropriate personal care skills and safety skills.

Assist the individuals with community inclusion, medical appointments, prescriptions, shopping, transportation, and third-party benefits and other tasks as needed.

Complete and maintain current required training status.

Maintains a collaborative work environment and works well with others.

Other duties as assigned.

Must meet at a minimum, one of the following requirements :

Have a bachelor's degree from an accredited college or university with a major in education, rehabilitative science, business or a related degree.

Have an associate’s degree or two years of college from an accredited college or university and have two years of documented direct experience with recipients with developmental disabilities.

Have one year of college from an accredited college or university and three years of documented direct experience in working with recipients with developmental disabilities.

Four years of direct professional experience in working with recipients with developmental disabilities may substitute for college on a year for year basis. The provider must hold a high school or GED diploma.

Must complete all Mandatory Trainings required by APD (Agency for Person with Disabilities from the State of Florida)

HIPAA from Train FL

Direct Care Core Competencies

HIV / Infection Control – Blood-borne Pathogens

CPR / First Aid (From American Red Cross, American Heart Association or Health & Safety Institute) CPR online training are not acceptable.

iConnect Train FL Trainings

SLC Pre-Service Certificate

Social Security Incentive Certificate

Any additional required APD trainings

We are a Central Region APD Provider. We are seeking Direct Support Professionals in Orange County, Osceola County, Seminole County, Polk County, Lake County, Hernando County, Highlands County, and Marion County.
