About the Job
This is a short-hour position working 16 hours per week, with a focus on case management and coordination of community resources. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient-centered care.
The successful candidate will work closely with our interdisciplinary team to assess patients' needs, develop and implement individualized plans of care, and provide education and support to patients and their families.
